Attention licensed Family Child Care Home Providers:
Do you know a family receiving subsidized child care from Child
Action, Inc. that wants to use your child care services? If so,
follow the steps below to enroll on our program.
Print, complete and submit these forms to Child Action, Inc.
You must also include:
- A copy of your pre-printed rate sheet which includes all of your rates and policies. This form must be type written and cannot be hand written.
- A copy of your Family Child Care Home License
The above information should be mailed to:

Once we receive the above information, you will be contacted by a Child Action, Inc.
Provider Department representative to complete your enrollment. You will be asked to
sign some additional documents at that time.
