• The date your attendance forms were received
• Your payment history

Contact our Provider Department at (916) 369-0191 to set up your login.

• For non-licensed providers, you must speak with a Provider staff person to verify your address, your State ID or Driver's License number and your SSN.
• For licensed FCCH providers, you must speak with a Provider staff person to verify your address, license number, and SSN or federal ID number.
